Do hangover and 'liver-support' pills work?

The verdict
The human-trial evidence that milk thistle, NAC, or turmeric shield your liver from alcohol is weak to absent — far thinner than the confident packaging suggests. A Cochrane review found milk thistle did not significantly help alcohol-related liver disease. The only thing that reliably lowers alcohol's harm is less alcohol.
What the pills claim — and what was actually tested
"Liver support," "detox," "recovery": the category sells the feeling of a chemical shield you can take before or after a night out. When you look for the randomized human trials behind that feeling, there's strikingly little — and what exists is mostly small, short, or in patients with established disease rather than healthy drinkers.
One supplement at a time
Milk thistle (silymarin): a Cochrane systematic review of randomized trials found no significant benefit for alcohol-related (or viral) liver disease. The headline ingredient of the category has the clearest "doesn't work" verdict.
NAC (N-acetylcysteine): genuinely used in hospitals — as an antidote for paracetamol (acetaminophen) overdose, under medical supervision. That is not evidence that a nightly capsule protects a healthy liver from alcohol, and it shouldn't be marketed as if it were.
Turmeric/curcumin: poorly absorbed by the body and without good human trials showing it protects the liver from alcohol. Promising in petri dishes is not the same as proven in people.
The move that actually works
The harm-reduction lever is unglamorous: less alcohol, less often. Food, water, and pacing can make the next morning easier, but they don't buy back the dose your liver and the rest of your body processed. And the old idea of a protective "healthy glass" looks weaker than it used to — Mendelian-randomization analyses suggest the cardiovascular case for any alcohol has been overstated.
